What keeps happening that shouldn’t?
A decision is made but does not hold. A programme repeatedly stalls. A leadership team agrees in the room and pulls in different directions outside it. Accountability exists everywhere on paper and nowhere when something fails.
You do not need to know the cause before getting in touch. The problem is the starting point—not the diagnosis.

We agree on the question the diagnosis must answer and set a clear boundary around the investigation.
02 — Trace
We follow the relevant decisions, records, handovers, meetings and points where progress stalls or changes direction. We speak to the people closest to what is happening.
03 — Diagnose
We compare what people say with what the evidence shows and identify the mechanism repeatedly producing the problem. If it matches a documented pattern, we name it. If it does not, we do not force it.
04 — Recommend
Every diagnosis includes a Path to Resolution: specific, evidence-based recommendations showing where intervention should begin, who needs to own it and what to stop, change, develop, streamline, automate, redesign or prioritise — and why. We prioritise the actions so you know what matters now, what comes next and what can wait.

The explanation has to survive contact with the evidence.
Depending on the question, the diagnosis may draw on:

meeting artefacts and follow-up actions
role accountabilities and delegated authority
programme plans, timelines and operating data
confidential conversations
observation of live work
the gap between formal process and actual practice
We do not begin with a preferred pattern and search for proof. The evidence decides.

the agreed diagnostic question and boundary of the investigation
a clear diagnosis of the mechanism producing the problem
the evidence supporting the diagnosis, including conflicts or uncertainty
the organisational patterns and contributing conditions sustaining it, including any Dandylion patterns the evidence genuinely establishes
the impact on time, money, delivery, capability, decision-making, trust and organisational risk, quantified where the evidence allows
a Path to Resolution with specific, evidence-based recommendations, prioritised by what needs attention first
the limits of what can responsibly be concluded
a findings session to take you through the diagnosis, evidence and recommendations and answer your questions
The purpose is not to produce another description of the symptoms. It is to make the underlying problem visible enough for a real decision.
When a diagnosis is appropriate
This work is useful when the explanation no longer fits the evidence.
A Dandylion Diagnosis may be appropriate when:
the problem has survived previous attempts to fix it
different parts of the organisation hold different explanations
a leader needs an independent account of what is happening
the formal process says one thing and the work does another
another intervention would be expensive without knowing the cause
It is not a culture survey, maturity assessment, strategy workshop or investigation designed to validate a conclusion already reached.
Scope
The question determines the scope.
Every diagnosis begins with a free scoping conversation about what keeps happening and what the organisation needs to understand.
If the problem is suitable for diagnosis, we agree the question, evidence required, access, output, fixed scope, price and timeframe before the work begins. If it is not suitable, we will say so.
Focused diagnoses can be completed in as little as 10 working days. Larger or more complex investigations are scoped upfront and are generally completed within 6–8 weeks. The agreed timeframe assumes access to the people, information and evidence required.
Diagnostic investigations start from NZ$6,000 + GST. Payment is 50% to commence and 50% on delivery, subject to any agreed procurement requirements.
Following the evidence needed to answer the agreed question is included. If the evidence reveals a genuinely separate issue that warrants an expanded investigation, we will discuss it with you first. No additional work or cost is added without your agreement.
